There's been another mockup of the Revolution controller "leaked" at the Nintendo Power forums, here it is.
This picture was posted at a topic called "Nee advertisement (56K)", and for some reason has been quickly deleted. Now that could just be related to something someone posted. But it may also be related to the mock-up itself. I´m not going to shout *woo woo* here, but the mock-up looks a likely contender. The ad´s style is what could be expected for a teaser campaign, while you can see enough of the controller to see this wasn´t knocked up in five minutes by some teenager on PhotoShop.
